NEW PATIENTS
If you're a new patient, you need to complete the initial intake forms. This will allow us to obtain demographic information and also a better understanding of your situation to develop tailored interventions.
Forms can be filled out directly from this website with any smart device. There are printable versions that you can download, complete and fax, email or mail back to us. The two corresponding documents need to be completed before the initial appointment. Additionally, we will send you a reminder text as a courtesy 24 hours before the date. You are encouraged to confirm your appointment as failure to do so can result in cancellation without further notice.
