GUIDELINES

Scheduling a Visit

To schedule a visit, you can contact our office by phone or through chat. The contact numbers are listed below. Please ensure you complete the Patient Referral Information Form.

Appointment Confirmation

The day before your appointment, you will receive a confirmation text message. Please respond to this text to confirm your availability. If we do not receive a confirmation, we reserve the right to cancel the appointment.

Insurance Verification

We make every effort to verify the insurance coverage of prospective patients. However, we strongly recommend that you contact your insurance provider to confirm your plan’s status and any co-payments. Please note that appointments may be canceled if insurance information is not provided at least one week before the meeting.

Requirements for Minor Patients

For patients who are minors, we require an initial intake session and regular contact with a parent or guardian throughout the treatment process.

First Appointment Preparation

For your first appointment, please complete the intake forms available on the New Patients page. These forms must be submitted before your initial visit to ensure we have all the necessary information to provide you with the best care.
