GUIDELINES

To schedule a visit, you can contact our office by phone or through chat. The numbers are below. Please complete the Patient Referral Information Form.

The day before the appointment, you will receive a confirmation text message. Please confirm this text as this will let us know that you will be available for the visit. We reserve the right to cancel an appointment that has not been confirmed.

We make every effort to verify the insurance coverage of prospective patients. However, it is recommended that you contact your insurance provider to confirm that your plan is active and any co-payments. Appointments can be cancelled when the insurance information has not been supplied at least one week before the meeting.

In addition, for patients who are minors we require an initial intake session and regular contacts with a parent or guardian throughout the treatment.
